This standard was reviewed during a DFPS investigation and found deficient. There were several people that stated that a caregiver is on her phone excessively throughout the day.
While doing a walk through, 2 classes had cleaning supplies that said keep out of reach of children under unlocked sinks and accessible to children.

There was a spring rider on the playground that requires loose fill that did not have loose fill. This was corrected at inspection.
Medication records were checked for the HASA and found that the full name of the children listed was not being used.

It was observed that diaper changing mat in infant room had mutliple tears. Packing tape was used to cover tears but came off when pad was cleaned.
During personnel records review one employee was found to not have current pediatric cpr and 1st aid.Cpr and 1st aid was for k - 12.
It was observed that a caregiver did not wash their hands with running water after an infant diaper change.
During inspection bleach wipes were observed in the reach of children. This was corrected at inspection when caregiver removed them.
During inspection the HASA was completed and two records did not have current immunizations.
During inspection it was observed a food allergy posting included five children. Records were reviewed and three stated diagnosed food allergies without a doctors statement or signature.
During inspection of the toddler 2's room a large hole behind the door was observed with loose drywall and insulation was visible and exposed to the children.
It was observed two infants were not strapped into the high chair table. This was corrected at inspection when cargiver strapped infants into the table.
It was observed after an infant's diaper change caregiver washed infants hands with baby wipes and not with soapy water.
