During the investigation it was determined a child was grabbed by the arm in 2024, the caregiver was fired but the operation failed to notify CCR of the incident.

During the investigation it was found that a caregiver pulled on a child's arm while disciplining in 2024. The operation fired the caregiver immediately and the caregiver was never rehired.

One classroom with children under 18 months was using the reduction ratio and out of ratio. This was corrected when the co-teacher came back in the room.

While reviewing file, 5 child files did not have signed Parent's Rights.

While doing the walk through, there were 2 classes with a total of 13 children not usiing safety straps while children were eating in a table.

Children immunizations were reviewed at inspection and two files were not current and up to date.

During the walk through the diaper changing pads in 4 of the classrooms had tears and could no longer be sanitized properly.

During the walk through there were 3 spots on the wall that were found that had pealing paint that the children had access to that could be pealed.
