Soiled diapers were observed in a trash can witihout a cover in a toddler room.

The director and a caregiver's training certificates in pediatric first aid with rescue breathing and choking and pediatric CPR were missing the expiration or renewal date of the training.

There was no documentation available for review for the unitary surfacing in the outdoor playground.

The Pre-K classroom's interest centers were not equipped with enough popular items available for the children.
