One caregiver hired on 1/20/25 completed Pediatric CPR training online without hands-on skill-based instruction, and practice.

There was no Emergency medical transport and treatment authorization forms for ten children who were transported from school. The fire extinguishers in the two transportation vehicles were not secured. One vehicle did not have the brackets to secure the fire extinguisher and the other vehicle the bracket was broken.

The eight-passenger minivan used to transport children did not have a child safety alarm.

There was a hole in the door of the school-age classroom, as well as a hole in the wall, where a child was observed picking from the damage.

3 out of 8 staff records reviewed were missing training hours.

1 out of 8 children's records reviewed was missing up to date immunization records.

Evacuation diagram observed in rooms not accessible for the children to review. Diagram was posted high in the wall where the teacher information board is located.

Updated fire inspection was not available for review. The latest fire inspection report was completed in May 2022.

The interest centers furniture was observed turned around facing the wall making it unattainable for children to make independent use of the material.

A staff member was observed providing care of children without having received notification from the Criminal Background Check Unit that this individual is eligible to be present at the operation. NOTE: This was corrected by staff member leaving the operation during the inspection.

Attendance records for director and one employee were not available for review during inspection.

Based on evidence gathered during the investigation, the Director was not regularly present at the operation to exercise director's responsibilities.

There were 5 infants under the care of 1 caregiver. Children's Specified Age Group was 0-11 months old. NOTE: This was corrected during inspection by person in charge stepping in as a second caregiver.

There were 25 children with a Specified Age Group of 5 years old under the care of 1 caregiver. Children's ages were between 4 and 12 years old. NOTE: This was corrected during inspection. 11 of the children were moved to another classroom under the care of a second caregiver.

Hand washing after diapering did not occur for a child in the toddler classroom. NOTE: Hand washing was completed after caregiver was informed.

1 out of 4 staff files reviewed did not have orientation statement nor pre-service documentation available for review.

Documentation of CPR and 1st aid training for 1 out of 4 staff records inspected was not available for review.

Documentation of fire drill for the month of June 2022 was not available for review.
