The staff administered expired medication to child. *Operation admitted this after realizing that it happened*

During the investigation process, it was determined a caregiver did not have administering medication training.

During the investigation process, it was determined that the parent did not sign the food allergy plan.

An infant not yet on table food did not have updated feeding instructions. This was corrected at inspection when updated instructions were sent to the operation.

The diaper-changing mat that was ordered does not meet the requirements or the needs of the children.

Children ages 14 months - 20 months were playing on a play structure over the allocated height required. The height was 46 inches.

During the walk-through, there was a 12-month-old child in the classroom with a 3-year-old. Exceeding the 18-month age requirements.

Diaper cream was expected in the toddler restroom. Diaper cream was moved to children's cubbies that were out of the reach of the children.

In one of the toddler classrooms, there was not a changing mate for diaper changing.

Two of eleven employee files reviewed did not contain education qualification verification.

The fire extinguisher's were overdue for annual service. During inspection the operation did not have record of monthly carbon monoxide/smoke detectors/fire extinguisher safety checks log.

During inspection, one of eleven employees had an expired CPR/First Aid certification.

A corner fence post on the PreK playground had deterioration at the bottom of the post, and pieces of wood were peeling off bottom of the post. Note: This was corrected at inspection when staff applied duck tape covering the deterioration and picked up the wood chips near the post.
