YMCA @ Westpark Elementary

During the inspection it was determined that the Food Emergency plans were not signed by both the parent and health care professional.

Evacuation diagram did not have the locations for severe weather shelter, designated location to meet outside.

The governing body has failed to ensure this location is operation in compliance the required minimum standards and laws.

The emergency medicaiton for a child with a food allergy plan was not on hand at the operation during the inspection.

At the start of the inspection at 3:40 pm. the three caregivers present did not know how many children were in care. Nine of the children had been in care since 2:30 p.m. It is unknown if any children left with parents prior to their being acconted for. The violation was corrected when the staff were asked to have all children group t gether so that a name to face be done and the children divided and documeanted with their group leaders. A caregiver assigned to this location site stated they were were not aware that child in care having a food allergy which required emergency medications.

One food allergy plan for chlid enrolled on 5/7/2024 was not signed by the parent or the child's physican. The medication stated ont he allergy plan was present at the oepration.

The permit holder failed to designate a site director within an extended period of time.

The program failed to notify Licensing that they were not providing before school care as they previously stated they were doing.
