Licensee states that she will ensure she and Director retake Mandated Reporter Training, read Reporting Requirements section 101212, and will provide written summaries of both to LPA no later than close of business on 10/11/21. In addition, she states that she will report incidents as required in the future. Child# 1 arrived at the facility with injury without parental explanation. Staff felt it was serious enough to report up the chain but failed to report to both the Dept and CPS. Child subsequently was hospitalized with extremely serious injuries which could have possibly been avoided if reported, which poses an immediate health & safety riskto children in care.

Based on interview with Director and file review, it was determined that Staff #2 and #4 did not have a TB test that was completed within on year prior to or 7 days after initial presence in the center. This is a potential risk to the health and safety of children in care.

Based on interview with Director and file review, Staff #2 did not have evidence of immunization against Pertussis or Measles on file. This is a potential hazard to the health and safety of children in care.

Staff did not report suspected child abuse to appropriate agencies.
