Based on interview with Director and file review, children under 12 months (Child #2 and 3) did not have an LIC 9227 on file. This is a potential hazard to the health and safety of children in care.

Based on interview with Director and file review, Children #2, 3 and #4 did not have current Infant Needs and Services Plans on file. This is a potential risk to the health and safety of children in care.

Child's Medical Assessment. Prior to, or within 30 calendar days following the enrollment of a child, the licensee shall obtain a written medical assessment of the child... This requirement was not met as evidence by: 1 Director stated that will ensure that all required documentation is completed and on file by the plan of correction date and proof of correction will be sent to Licensing. Based on interview with Director and file review, Children #1 and 3 did not have a medical assessment on file and the 30 days had passed.
