Based on LPA interview with the child mentioned in the complaint the teacher in fact handled her inappropriately. Interviews with other children in care revealed that when they don’t listen, the teacher in question gets upset, rude and yells at them, which poses an immediate Health, Safety, or Personal Rights risk to the children in care. Director will provide a copy of the training agenda along with the Staff attendee signature via email no later than 02/21/2023.

101238 Buildings and Grounds (a) The Child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well being of children , employee and visitors. This requirement not met as evidence by: 1 Director has made water fountain inaccessible and water is available to children. Water pitchers are available for supplying children with water in doors and outdoors. Based ointerview and record review, results of the Lead Water Testing conducted at the facility on 8/18/2022 were in exceedance, which poses a potential Health or Safety, or personal rights risk to persons in care. 8 Director will speak with the owner to determine if water fountain will be replaced or removed. Director will provide answer by 09/19/22 and final corrections will completed by 09/30/22.
