The director will submit a summary of the supervision training she conducted after the incident occurred and staff will complete a supervision handout that will be submitted along with the training summary to LPA by 4/17/2026. A 5 year old wandered away from the facility and was found crossing back and forth across the street by 2 motorist. The motorist returned the child back to the facility. 8 Subsequent zero tolerance violations are $1000 immediate civil penalty and $100 per day will be assessed until corrected

Director shall submit a plan on how to inproved active supervision of children in care while outside in the yard. The plan shall be submitted to CCL (LPA) by 11/7/2025 Daycare children were not adequately supervised during an incident where the children climbed a fence and threw rocks next door. LPA's also observed children not being visually supervised on 8/13/205 which poses a potential risk to the Health, Safety or Personal Rights to children in care. 8 ·Failure to correct will result in a $100 per day civil penalty until corrected. ·Repeat violations are $250 per violation and $100 per day until corrected.

(1) The licensee shall take measures to keep the center free of flies, other insects, and rodents. 1 Facility will immediately move all cleaning supplies to proper storage area away from children in care. Facility will be cleaned and items need to be stored in an organized manner. Trash will be removed daily to prevent insects. (2) The licensee shall safely dispose of water and any disinfectants/solutions that have been used for cleaning. This requirement is not met as evidence by LPA observations of flies in facility, cleaning products accessible to children, trash and the lack of sanitary practices throughout the facility 8
