Based on interview and record review, the licensee did not comply with the section cited above in that, C1 and C2 did not have their immunization records in file, which poses a potential health, safety or personal rights risk to persons in care.
Based on LPA’s interviews and record review, Director informed LPA Alyssa Kosky has been working at the facility since February 2025 and Heather Watson has been working at the facility since June 2025, which poses an immediate health and safety risk to the children in care.

Based on facility Lead sampling tests it was discovered that Outlets F, G, and H had high level of lead. This poses a potential risk to the health of children in care. 8 Director stated they will replace the faucets and are scheduled to retest on 12/13/22. Director will submit retest results by 12/31/22.
