Based on records reviewed and interview, The facility did not provide the 03/5/24 licensing report LIC809 and acknowledgement receipt LIC9224 to all families by the next business day. this poses an immediate risk to the health, safety, or personal rights of children in care. 8 children are in care, and to any newly enrolled parents/guardians for 12 months from the 3/5/24. Facility is to provide signed LIC9224 to LPA by8/26/25. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must also be placed in the child's file for verification.

Based on LPA observation, C1, who is enrolled in the toddler component was comingling in the infant room. This poses a potential risk to health and safety to children in care

101161(a) Limitations on Capacity and Ambulatory Status: (a) A licensee shall not operate a child care center beyond the conditions and limitations specified on the license, including the capacity limitation. The director stated that moving forward the toddler option will not be comingled with the preschool or infant rooms. Based on LPA's observation and director interview , the licensee did not comply with the section cited above by means of children fron toddler option being placed in the infant room which poses an immediate health and safety risk to children in care. 8

101221(a)The licensee shall ensure that a separate, complete and current record for each child is maintained in the child care center. The director stated that she will complete infant files and provided a statement via email to LPA. Based on LPAsrecord review, the licensee did not comply with the section cited above by means of not having a completed record for children which poses an immediate health and safety risk to children in care. 8
