Based on record review, the licensee did not comply with the section cited above in that Child 2 did not have a physician's report available for review which poses a potential health, safety or personal rights risk to persons in care.

Based on interviews, the facility did not comply with the section cited above in that interviewed staff reported to have witnessed or was aware that S1 showed a toy to C1 knowing C1 was fearful of that toy, and that S2 threw a soft ball at C1, which poses an potential health, safety, or personal rights risk to persons in care.

Based on observation today, the licensee did not comply with the section cited above in one count out of one total count of objects having 2 pieces of baby bouncers in the infant room which poses a potential health, and safety risk to persons in care. There were no babies in those equipment during inspection.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in one count out of two counts missing infant/toddler unit for Staff # 2 in the infant room which poses a potential health, and safety risk to persons in care.

Based on observation today, the licensee did not comply with the section cited above in 4 counts of out of total 4 counts missing labels either with the name or dates on the baby bottles and food borough from home which poses a potential health, and safety risk to persons in care.

LPA's observation and reviewing documents C2, C3, and C4 belonged to Toddler Option Program and were commingled with infants and toddlers. This is a potential risk to the health and safety of children in care. 8 The correction plan will be sent to LPA's email address by the due date of 9/17/2024.
