(a) The licensee shall provide care and supervision...(1) No child(ren) shall be left without the supervision of a teacher at any time... This requirement is not met evidenced by: 1 Site Supervisor had conducted individual meetings with involved staff to address concerns, and a retraining for all facility staff regarding Supervision Best Practices on 09/18/2024. Based on interviewes, all 3 interviewed staff confirmed that Child #1 was left unattended in the playground for approximately 10 minutes since Staff #1 and Staff #2 did not follow transition procedure, this posed an immediate risk to the health, safety and personal rights of the children in care. 8
