The director will provide a written statement plan. Based on staff interviews C1 was alone in the hallway until a teacher found C1. This is an immediate risk to the safety of the children in care.

Based on staff interviews, eight staff members reported that the facility is out of ratio during morning hours. This is an immediate risk to the safety of the children in care.
