Based on observation, LPA observed two (2) staff members using the restroom while one (1) child waited to use it. This poses a potential risk to the health, safety and personal rights to children in care. 8 with their Executive Director in possibly getting a waiver into the Department. Anita Holt stated she will review regulation and write a statement on what they have learned and submiit the statement to the Department by 4/11/2025.
Based on LPA record review and staff interview, Staff 1 last completed the Mandated Reporter Training on 02/22/2022 and has not been renewed. Staff #2 has not completed the Mandated Reporter Training. This poses a potential risk to the health, safety and personal rights to children in care.
Based on record review of staff files, all staff files were missing their immunization records for influenza (or declination for influenza), pertussis and measles. This poses a potential risk to the health, safety and personal rights to children in care.
