Based on observation and record review, the licensee did not comply with the section cited above which posed a potential health, safety or personal rights risk to persons in care. The facility ad their water testing on 6/23/23. However the faucet (hose) water feature on the little playground showed exceedance of lead. The facility took action and corrected the deficiency. Today LPA observed the pipe was cut from the base and there is no accessibility of water to children from this source.

Based on observation, and record review, the licensee did not comply with the section cited above in 4 counts out of 5 children's files missing the required forms of LIC 9227 for children under 12 months old which pose posed a potential health, safety or personal rights risk to persons in care.
