facility file review. Sign in and out sheets show that on 08/19/21, there were 8 toddlers present with 01 staff before 8:39 AM. This posed a potential Health and Safety risk to clients in care.
staff and other witness 1 Director states she will have an individual staff meeting with each staff in regards to active supervision. Director needs a week to complete the meetings.
staff and other witnesses disclosing that the kiddie gate in the toddler restroom area was broken. This gave children access to this area 1 Director replaced broken gate. LPA observed broken gate in the trash during the inspection on 10/01/21. Deficiency corrected during this inspection. and some children were hurt by it. This was a potential risk to the health and safety of children in care.
