101238 Buildings and Grounds (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement is not meet as evidence by: Based on interview with staff, documents reviewed and received and LPA 1 Director states her and the assistant director will ensure that the staff are doing there job as far as cleaning and will do follow up and update the cleaning check list. Director states will submit what they will be doing in writing and send copy of update check list, by the POC date. own observation, It has been determined that the school age bathroom does smell of urine and the carpets and floors are dirty. This poses a potential risk to the health and safety of the children in care. 8

Based on interviews conducted school age children are calling each other nick names that some children don't like their nick name and it hurts their feelings and makes them cry. Licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.
