Director states they will submit to the department proof of disiplinary action for all staff involved as well as provide crisis prevention training to all staff, and take courses provided by the licensing department and provide a written summary from staff #1, #2, and #3. Per Interviews with the staff #1 and staff #2 and #3, all three admitted that they did lie to the police and to the department regarding the actual story of what happened regarding the child being left on the bus. This poses an immediate health and safety risk for children in care.
LPA observed two of the van/bus that are used to transport children, the registration has expired as of September 2022. LPA observed in the outdoor play yard black tarp below the wood chips 1 Supervisor states will have the yard cleaned and fixed by the POC date. Supervisor states will submit a letter with the details of how she will have part of the yard closed while being fixed and how the ratio will be met outside by using a staggered schedule during the correction period. is protruding in several areas of the yard and nails protruding from the green fake grass area and the wood surrounding it is warping. Yard has trash and the trash can is over flowing This poses a potential risk to the health and safety of children in care.
Director sent LPA copies of mask wearing posters she has placed in and around the facility as well as had all staff sign and date a statement regarding wearing a mask in the facility. Director will update the signed statements to reflect that they will wear mask and will wear them properly. Director will also update the signed document to reflect Per LPAs own observation of some of the staff wearing their mask under their nose, including the assistant director and per interviews with staff, some stated their mask slip off their noses and its hard to keep it on completely. This poses a potential risk to the health and safety of children in care 8 that this is a requirement by the California Department of Public Health (CDPH) Guidance on the Use of Face Covering and a personal rights issue under title 22 regulations.
