Per center director, the staff member is on administrative leave.Licensee will submit a written statement to community care licensing, summarizing what action has taken to ensure that the staff member will not allow further incident to occur by 6/23/26. INTERVIEWS WHICH REVEALED 5 & 6 YEAR OLD BOYS/GIRLS WERE ALLOWED TO CHANGE CLOTHES IN THE BATHROOM AT THE SAME TIME FOR A WATER PLAY ACTIVITY. THIS POSES UNCOMFORTABLE ACCOMMODATION FOR CHILDREN IN CARE.

licensee will submit a written plan on how staff will encourage children's safe play on the climbing structures and wil alsol conduct a staff training on safe play for toddlers. licensee will submit the plan and a signed list of staff that attended training to community care licensing by 5/12/26 a tour of the facility, interviews and record reviews which revealed a 2 year old child ran onto a play structure, fell and injured themselves. Staff did not discourage the child from running onto the structure which is meant for climbing only.

INTERVIEWS AND A REVIEW OF RECORDS WHICH REVEALED ON AT LEAST ONE DAY A TEACHER WAS SUPERVISING MORE THAN 12 CHILDREN WITH NO ASSISTANCE 1 On this day, the assistant director went into the room to bring the class into ratio. The licensee will submit a plan of action to community care licening by 9/13/24 to ensure that ratios are met at all time in classrooms in the am and at the end of the day.

INTERVIEWS WHICH REVEALED A STAFF MEMBER HAS USED A HARSH TONE WHEN SPEAKING TO CHILDREN IN CARE 1 Licensee will have the staff member review the personal rights videos on the CCLD website and write a summary on personal rights and to communicate with children. Licensee will submit the summary to CCL by 9/19/24

Interview and record reviews which revealed the early preschool classroom was out of ratio for a period of time on 7/11/24. 1 Per director, after notification of the room being out of ratio, a staff member went into the room to bring the facility into compliance. Licensee will submit a written plan of action to ensure the proper ratios are kept at all times. Licensee will submit the plan of action by 7/25/24
