(1) Each infant shall be constantly supervised and under direct visual observation and supervision by a staff person at all times. This requirement was not met as Licensee will require all infant staff to participate in supervision training on 02/16/2026. Licensee will email LPA the agenda for the training by 5:00 PM on 02/13/2026. evidenced by a child exiting his/her classroom alone on 02/09/2026. The child was without visual observation for approximely 30 seconds. This poses/posed an immediate health, safety, or personal rights violation to the person(s) in care. 8
Based on record review, the licensee did not comply with the section cited above when 1 staff member was providing care and supervision to children without a background transfer to associate to the license which poses an immediate health, safety or personal rights risk to persons in care.

All staff members will read and sign the new playground policy and schedule. The policy and schedule will be emailed to LPA by 5 pm on 12/11/2023.
LPA observation of playground surface and interviews. 1 The facility will repair or replace the surface on the infant playground by 12/01/2023. Until such time, the facilty will provide outdoor play for infants which does not comingle with other age groups. LPA will conduct an unannounced POC visit to clear the deficiency. The unsafe condition of the infant playground surface poses a potential health, safety or personal rights risk to persons in care.
