Supervision shall include visual observation. 1 Director will implement a new policy for entering and exiting the building from the outside play area. Director will have all preschool staff sign the new policy and will email LPA the signed policies by COB on 09/10/2026. This requirement was not met as evidened by 2 children walking into an empty classroom on 08/31/2026. The children were without visual observation for approximtaly 2 minutes. The lack of supervision poses/posed a potential health, safety or personal rights risk to persons in care. 8

Director Roberts will email LPA Velazquez the agenda with the date of training by no later than 5:00 pm on 06/22/2026. only twice during 7 hours and 50 minutes of care on 06/9/2026, only 3 times during 7 hours and 51 minutes of care on 06/10/2026, and only twice during 7 hours and 32 minutes of care on 06/11/2026. The policy will remind staff members that they may not supervise more than 12 preschool aged children alone and this may require not allowing children to be signed in if the classroom is at ratio with 1 staff member. D1 will email the signed policies to LPA by 5:00 pm on 05/30/2025.

T1 pushing C1, on 09/12/2024, which 1 Director Roberts (D1) will ensure that the entire preschool staff (and Toddller Option staff) receive training in Personal Rights, which will include the CCLD video "Children's Personal Rights in Child Care." D1 will email LPA the details on what the training will entail and the date in which the resulted in C1 falling. This poses/posed an immediate risk to the health, safety, and personal rights of person(s) in care. 8 training will occur by 5 pm on 10/29/2024. When the staff completes their training, D1 will ensure that a signed attendance roster is completed.

When S1 completes the training, Licensee will ensure that a signed statement is in S1's file aknowledging that S1 completed the training.

During today's visit LPA observed that the affected water outlets are not in use. Director will have the sinks repaired and the water re-tested. Director will report the new results to LPA. The water report dated 10/06/2022 revealed that the facility had elevated levels of lead in 4 water outlets. This is a potential health and safety risk to the children in care.
