S1 grabbing and yelling at C1 on 1 Director Roy (D1) will create a form that all substitutes and new staff will sign, aknowledging childrens' personal rights, before they provide care and or supervision to children. D1 will email LPA the form for review by 5:00 PM on 05/02/2025. 04/23/2025. This poses/posed an immediate health, safety or personal rights risk to persons in care.
S1 yelling at C1 during potty training, 1 All staff members will receive training in children's personal rights. The training will include the CDSS video "Children's Personal Rights in Child Care," available at CDSS.ca.gov. All staff members will sign that they received training, and the attendance log and after C1 had just incured a slight injury. S1 was within proximity to C1's face. S1's action(s) poses/posed aa immediate health, safety or personal rights risk to persons in care. 8 will be emailed to LPA by 5PM on 02/19/2025.
LPA observing Desiree Hodge providing care and supervision to children on 08/22/2024. Record review revealed that Desiree Hodge was not associated to the facilty. This poses/posed an immediate health, safety or personal rights risk to persons in-care.
D1 stated that the faucet will be remediated or replaced and the water re-tested. D1 understands that the results of the re-testing must be emailed to LPA. The water report dated 03/27/2023 revealed that the facility had elevated levels of lead in one water faucet in the kitchen. This is a potential health and safety risk to the children in care.
