Based on LPA's observation, the licensee did not comply with the section cited above and tarp in the entrance of sand area could be a tripping hazard and wooden seating area around the sand area the paint was chipped and needed to be repainted which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on observations , records review and interview , the licensee did not comply with the section cited above. Staff #3 was missing proof of pertussis and Staff #9 was missing measles immunizations , which poses/posed a potential health, safety or personal rights risk to persons in care.
