Based on observation and record review, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. LPA observed S1, S2, and S3 did not have a current mandated report training which were valid till 3/23/22, 3/24/22, and 4/22/20. Director stated that staff forgot to completed a current mandated reporter, and thought it needed to be done once.
