Based on observation and record review, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. LPA observed S1 did not have a current mandated reporter training which was valid until 12/20/20. Director stated that staff forgot to completed a current mandated reporter, and thought it needed to be done once.
