Based on interviews and S1's admission, the licensee did not comply with the section cited above in that it was determined on 11/7/25 S1 used inappropriate discipline to C1 and C1’s Personal Rights were violated which poses an immediate health and safety risk for persons in care. 8 Director will implement the CCLD video of Children’s Personal Rights; https://ccld.childcarevideos.org/child-care-center-operators/childrens-personal-rights-in-child-care/ Director will submit the attendance sheet and agenda to the SDRO by 11/24/25.

Based on interview and record review, the director did not comply with the section cited above in that S2 did not have a current, valid mandated reporter certificate, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on interview and record review, the director did not comply with the section cited above in that S1 had not completed one required educational course, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPA observation, staff did not ensure facilty playground was safe and in good repair. LPA observed, peeling paint, large splinters, protruding wire, and cracked and lifted cement in the 5 year old playground, which poses a potential health and safety risk to children in care.

Based on record review, Director did not have LIC 503 Health Screening Report available in two staff files which poses a potential health and safety risk to children in care.
