Based on interviews and S3's admission, the licensee did not comply with the section cited above in that it was determined on 6/18/26, S3 used inappropriate discipline and violated C1’s Personal Rights, which poses an immediate health and safety risk to clients in care. 8 Director stated S3 was terminated on 6/22/2026 and no longer works at the facility. Director will submit the attendance sheet and agenda to the SDRO by 8/17/26.
Based on interviews conducted and record review the licensee did not comply with the section cited above as the facility had damaged window screens and evidence of a prior water leak that resulted in a musty odor which poses a potential health, safety or personal rights risk to the children in care. 8 The facility will provide documentation confirming that staff who attended the meeting acknowledged the information presented. This report is an amended version of the original report created on 7/23/2026.
101216.3 (a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance. This requirement was not met as evidence by.. 1 Lead Teacher Brianna Bustos, stated that Director will conduct a staff meeting to discuss teacher-child ratios, emphasize the importance of maintaining proper documentation, and reinforce the requirement to notify upper management whenever assistance is needed to remain in compliance with ratio requirements. Based on interviews and record review the facility did not comply with the section above as on May 12, 2026 transition logs reflected that there were 25 children supervised by one (1) staff member for approximately 25 minutes, resulting in the classroom operating out of ratio during that time. 8 The facility will provide documentation confirming that staff who attended the meeting acknowledged the information presented. 101229(a)(1) Responsibility for Providing Care and Supervision. The licensee shall provide care and supervision as necessary...No child(ren) shall be left without the supervision of a teacher at any time...Supervision shall include visual observation. Director stated all staff received In-Service training on the importance of face to name transition on 10/28/25. Director stated a new procedure has been implemented as of 10/28/25 requiring staff to communicate with a supervisor, by either walkie talkie or text, Based on interview and record review, the licensee did not comply with the section cited above in that it was determined that on 10/27/25 there was a Lack of Supervision resulting in C1 being unsupervised, which poses an immediate health and safety risk for persons in care. when transitioning from the playground back to the classroom. Supervisors will physically walk around the playground ensuring all children are accounted for.
Based on LPA interviews conducted, the licensee did not comply with the above regulation, as incidents reviewed indicated multiple bite injuries with children as a result of a lack of supervision by staff, which poses an immediate Health & Safety risk to children in care. 8 for staff addressing prevention of injuries, utilizing the CCLD training video: Supervising Children In Childcare Centers https://ccld.childcarevideos.org/child-care-center-operators/supervising-children-in-child-care-centers/ and other resources. Director will submit the agenda and sign in sheet for the training to the SDRO by 3/28/25.
Based on LPA interviews conducted, the licensee did not comply with the above regulation, as staff neglected to notify C1’s parents of at least one (1) injury incident in a timely manner which poses a potential Health & Safety risk to children in care. 8 reported to the child's authorized representative and recorded in the child's record. Director will submit the agenda and sign in sheet for the training to the SDRO by 3/27/25.
Based on review of video footage, staff interviews, and facility record review, at the time of this incident facility staff did not ensure the safe and proper handling of children which resulted in injury to a child that required medical attention and posed an immediate Health and Safety Risk to children in care.
Based on observation, interview, and record review, facility did not ensure a safe enviroment for the activities planned by allowing children to ride tricycles on the preschool playground near a surface that sloped downward towards a metel shade canopy, which posed a potential health and safety risk to children in care riding tricycles.
Based on interviews and record reviews, staff failed to notify C2's parent of the incident which poses as a possible risk to children in care.
