Based on interviews, photographs and incident reports, the licensee did not comply with the section cited above in that it was determined for a two-month period C1 received multiple bites while in care and C1’s personal rights were violated which poses a potential health and safety risk to clients in care. 8 Director will submit the attendance sheet and agenda to the SDRO by 8/17/26.

Based on observation and interview, the licensee did not comply with the section cited above in that two out of three infants bottles did not contain the date which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPA observations and interviews conducted, the licensee did not comply with the above regulation, as facility is not maintaining an appropriately equipped isolation area for ill infants which poses a potential Health & Safety risk to children in care. 8 provided to staff to review the new procedures. A copy of the updated Plan of Operation, training curriculum, and staff in-service attendance sheet, along with photos of the area and equipment will be provided to SDRO by 2/7/2025.

Based on LPA/LPM observation, Interview of staff and record review, the licensee did not comply with the section cited above, Three infants who staff reported were able to roll from back to stomach did not have section C completed of the LIC 9227. One of the three infants was observed sleeping on their stomach which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, interview and record review, the licensee did not comply with the section cited above and the LIC 9227 was missing in five children's files which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on water testing results and interview, facility tested over the Action Level Exceedance level at two water outlets (spigots). This may pose a health, safety or personal rights risk to children in care

Staff do not prevent child from being bitten by another child.

Staff do not ensure children showing symptoms of illness are isolated from other children.
