(a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance, except as specified in (b) and (c) below. Based on the observation, the Licensee did not meet the above regulation which poses an 1 Director understands there shall be a ratio teacher-Child ratio of 12:1. Director agrees to submit a plan of action to ensure all classrooms are properly staffed according to their attendance. Plan of action is due on or by close of business on 5-13-2026 immediate Health, Safety & Personal Rights risk to the children in care. During the facility tour, the LPA observed one Teacher with 15 children on the playground while the Co Teacher was in the hallway restroom with one child. 8

(f) Cribs shall be free from all loose articles and objects, including blankets and pillows. Based on the observation, the Licensee did not meet the above regulation which poses a potential Health and Safety risk to the children in care. During the facility tour, the 1 Director understands cribs shall be free of loose objects, including blankets and pillows.
