... Ms. Gonzales stated they are having a staff meeting Thursday 9/26 and will add personal rights and potty-training procedures to the training. An agenda will be sent along with staff sign in sheet via e-mail to LPA Lane on 9/30/24. Based upon staff interview, a daycare child was sent home without a pullup or underwear under the clothes which is a potential health, safety and personal rights risk to children in care.

... 1 Director stated that child C1 is no longer enrolled at the facility. DIrector will provide a written plan to help mitigate challenging behavior in the classroom that supports teachers and prevents multiple injuries from occuring to students and staff. Based upon LPA observation and interviews with reporting party, staff members, parents and children, there was not enough staff supervising to meet the children's needs which is a potential health, safety and personal rights risk to children in care.

... 1 Assistant director stated she submitted a work order for emergency pest control service to address the ant situation at the facility (and in clownfish room) and send copy of pest control report via email to LPA Lane by 9/24/24. Based upon LPA observation of ants in Clownfish room and staff interviews, facility has not addressed the ant issue adequately which is a potential health, safety and personal rigths risk to children in care.

Licensee states will provide staffing schedule that shows adequate staffing for the Sea Stars classroom no later than close of busienss 3/13/23, and will, in future ensure that all classrooms abide by capacity and ratio requirements. Base don observation and record review the licensee did not comply with the section cited above as the Sea Stars room had 19 children supervised only by one fully qualified teacher and one aid with out six units which poses an immediate health, safety or personal rights risk to persons in care.

TEACHER'S AID QUALIFICATIONS:An aide shall work only under the direct supervision of a teacher. 1 LIcensee states will complete a schedule that ensures that in each classroom a teacher's aid shall work only under the direct supervision of a fully qualified teacher and will provide this schedule to LPA no later than close of business 3/17/23. LIcensee also states will ensure that all classrooms in future are properly staffed. Based on observation and record review he licensee did not comply with the section cited above as eight infants were supervised by an unqualified teacher and an aid which poses a potential health, safety or personal rights risk to persons in care. 8
