On 10/31/24, at 10:55AM, during transition from the 7/8 classroom to the preschool yard, Staff 1 did not follow proper transition tracking procedures leaving Child 1 sleeping inside the classroom. This poses an immediate health, safety, and personal rights risk to children in care.
Lead water testing samples taken on 11/11/2022 revealed that levels of lead were beyond the allowable amount deemed by the State of 1 Per Director, the water outlet (N), was immediately decommissioned and tapped “Do not use”. Per Director, replacement of the location fixture has been replaced on 11/27/2022. California. Water Outlet “N” has Action Lead Exceedance (ALE) “N” 8.36ppb. This is a type B violation which poses a potential Health, Safety or Personal Rights risk to persons in care. 8 Director agreed to submit a written declaration of plan of correction, no later 12/16/2022.
