(a) The licensee shall ensure that each child is accorded the following personal rights: (2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. (3) To be free from corporal or unusual punishment, infliction of pain, 1 infant staff will review perosnal rights addendum. The signed addendum will be submitted to the department. Based on interviews and documents collected. The above regulation was not met C1 received a injury while attending the CCC on 3/4/2025. Mutiple witnesses osberved C1 in discomfort after waking up from nap. Aditionally Staff sated, they should have notified management sooner. 8

Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event. Epidemic outbreaks. The Director will review CCR Titile 22 101212(d)(1)(E) and submit a statement via email veifying that she fully undertstands the regulation for reporting requirements. german.negrete@dss.ca.gov This regulation was not met due to facility failing to report multiple cases of RSV out breaks to the department as required. 8

(h) Infants who are unable to hold a bottle shall be held by a staff person or other adult for bottle feeding...infant able to hold his/her own bottle shall be unbreakable. This requirement was not met as evicenced by: LPA observed a staff 1 Staff shall immediately ensure an infant whom is not able to bottle feed themselves is held for feeding. Director will review the regulations in care and ensure that they understand the requirements. bottle feeding infants while they were laying on the floor. Based on LPA observations, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. 8

(b) There shall a ratio of one teacher for every four infants in attendance. Facility will take training through the CCLD website videos and then have all infant staff sign that the training videos were done and submit to LPA Rios by 5/19/2023 or mail it into the office. Based on interviews obtained from all staff, the facility failed to ensure proper ratio of children which poses a potential health, safety, or personal rights risk for children in care. 8
