(a) The licensee shall ensure that each child is accorded the following personal rights: (2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. The centter will implement a new policy that will provide details on the steps staff will take to document challanging behaviors. This policy will also include steps center management and staff will take to ensure the safety of chidlren and staff, Based on interviews and documents collected the above mentioned regulation was not met, C#2 dispalyed agressive behaviour towards other children in care. There were multiple incidents, which includes a 2/22/25 incident where C#2 kicked C#1 in the face one report stated it was accdientally another report stated it was not accidentally. 8 when a child is displaying challenging/agressive behaviour in the center/classroom.

(m) All play equipment and materials used by children shall be age-appropriate. record review, revealed this requirement was not met: On 11/18/2024 at 10:12AM during outdoor play, C1 was walking on the obstacle course when C1 lost C1's balance and hit C1's head on a wooden log. 1 Director will sign a statment, confirming she understands the aformentioned regulation cited(fictures ,furnitures, equipment and supplies). Director will email the signed docuemnt to German.Negrete@dss.Ca.gov It caused bleeding and abrasion the the right side of the forehead. 911 was contacted and C1's abrasion required stiches. Facility could not provide manufacturer instructions for all materials(wooden logs) that c1 interacted with the day of the incident. 8

(a) The licensee shall provide care and supervision as necessary to meet the children's needs.(1) No child(ren) shall be left without the supervision .... this regulaiton was not met .. 1 The staff involved in the incident on 7/30/2024 will have to watch a video pertaining to care and supervision at child care centers. 101238 (g) Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children shall be stored where inaccessible to children. This requirement was not met as evidence by: 1 Facility will submit a written Plan of Correction on how they will prevent this from happening again to LPA Rios at austin.rios@dss.ca.gov by 6/24/2022. Facility will also partner with Ventura County Inclusion Program for further staff training. Based on observation and interviews C1 had access to a thin bolt like washer. This poses a potential health and safety risk to children in care. 8
