based on record review, video footage review, and interviews conducted facility failed to provide adequate care for C1. 1 The facility conducted staff training on 5/28/26 regarding to care and supervision and children’s personal rights. LPA obtained the training materials and sign-in sheet as proof of attendance. POC was cleared during today's visit. At 11:08AM during outdoor area, S1 picked up the child 1 (C1) by the right wrist then lifted C1 body above ground a few steps then dropped C1 on the bench. C1 observed to be unhappy which poses a potential health and safety risk to children in care.
