Based on observation, interviews, and records reviewed, the interim director was not present at the facility as required and no proper arrangements were made to have a fully qualified teacher assist in their absence posing a potential health, safety, or personal rights risk to children in care.

Based on record review, the licensee did not comply with the section cited above in 6 out of 7 children's records which poses a potential health, safety or personal rights risk to persons in care.

Based on staff interviews the licensee did not make sure the correct baby bottle was given to the correct infant. This poses a potential risk to the health and safety of children in care.

Based on observation and staff interviews, the Infant room ratio was observed to be 2:9, nine infants supervised with two staff. This poses a potential risk to the health and safety of children in care.

Based on Staff interview, on 9/30/22 a one year old child crawled out of his classroom and was unsupervised for approximately 37 seconds. This poses a potential health and safety risk to children in care.

Based on record review staff, Stephanie Saldarriaga has completed 9 units in ECE however, she is missing units in Infant Care and Curriculum. Ms.Saldarriaga is currently not enrolled in any ECE class.

Based on documentation received on 9/29/22, the facility failed to submit correction in a timely manner.

Director stated that the playground safety procedures were reviewed again on the same day with the involved staff. Face to name procedure was reviewed. Director will provide training with staff members and she will submit the training agenda along with the staff members sign in sheet to Licensing no later than 9/19/22. On 8/31/22, an infant was left behind at the playground area for approximately fifty seconds, while the rest of his class was back to the classroom. This poses a potential health and safety risk to children in care.
