Upon the occurrence, during the operation of the child care center of any of the events specified in (d) (1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours..... 1 Consultation was completed during the visit with Administration of Yolanda Orozco and Lucy Barcelo. They were reminded of the procedures of reporting and if unsure of what is reportable to call the Department and consult with the On-Duty Analyst. This requirement was not met as evidence to record review of Incident Report and Unusual Incident Report (UIR) was dated that incident occured on 3/6/2024 and notification to the Department was on 3/18/2024 which poses/posed a potential health, safety or personal rights risk to persons in care. 8

(a) The licensee shall ensure that each child is accorded the following personal rights: (2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. Administration had meeting with parents to revisit behavior plan that was already in place for Child#1. Admin is consulting with LACOE to suggest classroom strategies. Child Dev. Director will have a staff meeting to consult safety measures and will provide LPA with itinerary and This requirement was not met as evidence due to interviews with staff regarding an incident of a child gaining access to the parking lot on 3/6/2024, which poses/posed a potential health, safety or personal rights risks to persons in care. 8 signatures of staff in attendance. LPA and Program Manager consulted regarding walkway used by parents and children to be avoided as an area for children and staff to utilize if a child needs a break due to behavioral concerns.

Testing results with fractional ppb readings of 5.5 ppb or greater shall be rounded up to the nearest whole number, before comparing to the Action Level. This requirement was not met as evidence by record review. 1 Per Program Manager children use Sparkletts Water service located inside each classroom for drinking water using dixie cups and children have constant access. Also, Meals and snacks are brought in by the Bassest Unified School Distict Child Nutrition Services. Lead test results showed that fountains in the classroom and outside the playyard had an action level exceedance. LPA observed water faucets to be made inaccessible via plastic cover. 8 The facility will provide a written statement on their plan to continue to ensure that the water sources will not be used as a drinking by the POC date of 12/12/2022.
