Based on LPA’s video review of the incident and interviews conducted, staff roughly handled child in care. This is an immediate risk to the Health, Safety and personal rights of children in care.
Based on interviews conducted and documentation received the facility did not provide a healthful and comfortable restroom evnviroment to meet the child's needs. This is a potential risk to the health and safety and personal rights of children in care. Deficiency Dismissed

Upon arrival to the facility, LPAs conducted a physical inspection of the facility. In Classroom N.2, LPAs observed Staff N.1 and Staff N.2 providing care and supervision to 12 children in care. 1 Facility director removed all items from Classroom N.2 and Classroom N.5 within this inspection. A written agreement plan must be submitted to CCL by COB 11/04/22. Director/Administrator agrees to provide a plan to have staff items ( which includes medication) inaccessible to children in care. Children were eating snacks and walking around the room when LPAs observed Staff N.1 's purse open and accessible to children in the room with prescription medication.This poses an immediate risk to the health and safety for children in care.
Based on records review and observation, facility did not comply with the section cited above in which LPAs observed three staff members present and working at the facility without an Eligible Criminal Record Clearance. Staff N.1-N.3 This poses an immediate health, safety, and personal rights risk to children in care. During this inspection, the facility called CCL 951-782-4200 to verify pending fingerprints and processing. A civil penalty is being assessed. Staff N.1 and Staff N.2 left during the facility during this inspection. Facility called Guarding and cleared Staff N.3 Licensee understand that Staff N.1 and Staff N.2 can not return to work until clearance has been confirmed.
Licensee/facility representative agrees to review staff records prior to employment and attach course descriptions to verify Title 22 compliance. Director agrees to follow up with UCI Child Development to provide a course description for completed college courses to verify Teacher/Teacher Assistance compliance. During this inspection, Staff N.1 was observed working in Classroom N.2 as a lead teacher providing care and supervision. However, staff records revealed that S#1 has college credits; however, courses in transcripts do not qualify S#1 to work as a qualified Lead Teacher and/or qualified Teacher Aide.
