Based on LPA's interview with Staff 5 interview and LPA's observations of the facility operating out of ratio (1 teacher with more than 12 preschool children) which posed an immediate risks to the health, safety and personal rights to the children in care. 8

This requirement is not met evidenced by: 1 Director states she will submit a detailed plan to ensure staff take appropriate actions to meet the children's need on the playground, to LPA by due date. Based on LPA interviews and record review, S1 failed to provide care and supervision as necessary to meet C1's safety needs resulting in C1's injury after jumping off the play structure multiple times. This posed a potential safety risk to children in care. 8

This requirement is not met evidenced by: Director will submit a detailed plan to ensure all incidents are being reported according to Title 22 Reporting Requirements, to LPA, by due date. Based on interview and record review, Director confirmed that due to miscommunication, an incident which occurred on 03/11/2024 was not reported to CCL until 04/25/2024, this posed a potential health, safety and personal rights risk to the the children in care. 8

The director stated moving forward they have now implemented putting the main teacher assigned to the classroom be present during mealtimes. food to which the child’s records indicate they are allergic. This poses an immediate health and safety risk to children in care. 8

This requirement was not met evidence by: 1 The director stated that she will be giving children in RM10 (Kinder) the option to rest and relax as needed. Then for the other classroom, the director stated she will have enough staff to do quiet activities with other children. Based on LPA's observation during the visit conducted on 9/12/2023 it was observed that children were layin in their beds awake during the facilities "nap time". Children were not observed to have additional activities provided to them and were observed to be in their mat or redirected to their mat. 8

Based on review of video of an incident, 1 The facility reported the incident to Licensing office within 24 hours, and has trained all preschool staff on conducted name to face procedure both indoors and outdoors when transitioning. Additional attention is being payed to child #1, most notably when transitioning and possibilities of hiding from staff at that time. Proof of this correction was LPA determined that a child #1 was in a classroom #7 unsupervised for 90 seconds. This is a potential threat to the children's health and safety. 8 gathered at the time of today's inspection on 8/5/22.
