Based on record review, the licensee did not comply with the section cited above in that the facility's last documented emergency disaster drill was from 07/17/2025 which poses a potential health, safety or personal rights risk to persons in care.

(a)(1) An aide may be substituted for a teacher when all of the following conditions are met: (A) There is a fully qualified teacher directly supervising no more than 12 infants... This requirement is not met evidenced by: 1 Facility Representative states they will submit a written plan of how the facility plans to maintain ratio at the facility at all times to LPA via email by due date. Based on LPA's interview with assistant director and LPA's observations of the facility operating out of ratio (2 unqualifed teachers caring for 8 infants) which posed an immediate risks to the health, safety and personal rights to the children in care. 8

101416.2 Infant Care Teacher Qualifications and Duties (b) Prior to employment, an infant care teacher shall have completed... three... units related to the care of infants... This requirement was not met evidenced by: 1 The facility representative stated they will only assign properly qualified teachers to supervise children alone. Based on LPAs interviews and record review, staff that are in the infant classroom do not have units in infant/toddler courses. This poses a potential risk to the health/safety of the children. 8

LPA conducted inspections on 09/23/2025, and 10/27/2025 and there was no director. Staff informed LPA that director is out on medical leave.

Unqualified staff providing care and supervision.

Director is not on the facility premises during the hours of operation.
