Director agrees to submit Plan of Correction (POC) to CCL-Fresno Office by 01/09/2026. The POC is to include proof of training materials pertaining to Title 22 Regulations-Children's Personal Rights and attendance (sign-in sheet) of staff participation. During the complaint investigation, it was revealed that Staff #1 (S1) grabbed Child #1 (C1) by the shoulder while attempting to prevent C1 from leaving the classroom. This action resulted in S1 scratching C1’s neck and leaving visible marks. This posed a potential health, safety, and personal rights risk to children in care.

(a) The licensee shall ensure that each child is accorded the following personal rights: (2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. This requirement was not met evidence by: Information was received that Fresno County Police Department was at the premises investigating an incident. 1 Per Director, she will work with Licensee to complete training material and to conduct a training for staff regarding children’s personal rights and different approaches of working with children. Director agrees to submit proof of training materials and attendance proof containing date, time and staff who attend the training. The proof is to be submit to the Department by 06/19/2024. During the unannounced Case Management-Incident conducted by LPA, it was disclosed that Staff #2 (S2) had put her upper body and arm over the children multiple times as an approach to have children falls asleep during napping time. This poses a potential health, safety, or personal rights risk to children in care. 8
