Based on interviews Staff 1 hit Child 1 on the hand while in care which is an immediate health, safety, or personal rights risk to the persons in care. 8

No child(ren) shall be left without the supervision, including visual observation, of a teacher at any time except as specified in sections 101216.2(e)(1) and 101230(c)(1). It was reported that a T1 and T2 left two children outside unattened for a short period of time. No injuries were This is a violation to the health, safety or personal rights of the children in care. 1 The facility will conduct training on Wednesday March 27th. Per director she intends to send LPA a copy of the training agenda along with the sign in and out sheet. The facility will send all the required documents to LPA Alemoh no later than the 29th.

This requirement was not met as evidence by: Based on evidence obtained during interviews conducted it was disclosed child #4 fell resulting in scratches on his nose and upper lip. 1 Director shall submit a written statement to the Department no later than 8/4/22 indicating what steps will be taken to prevent this type of incident from re-occurring. Supervising staff #3 and 4 were not aware of how child #4 sustained the scratches. This is a Type B deficiency that poses potential threat to the health and safety of children. 8

This requirement was not met as evidence by: Based on interviews conducted it was determined that on 07/08/22, child #1 fell from the play structure and landed on wood chips resulting in child #1 sustaining 1 Director shall submit a written statement to the Department no later than 8/4/22 indicating what steps will be taken to prevent this type of incident from re-occurring. scratches one on right above the eyebrow area, another one on the cheekbone and 2 scratches on the groin area. Staff #1 and #2 were providing supervision to a group of children (no more than 24 children). Staff #1 and 2 were not aware how child #1 fell off the play structure. This is a Type B deficiency that poses potential threat to health and safety of children in care. 8

Criminal Record Clearance: All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility: Obtain a California clearance or a criminal record exemption as required by the Department. This requirement was not met as evidence by: 1 Staff #2 is now fingerprint cleared and assiociated to the facility. Director will submit a written statement to the Department no later 7/15/22 indicating staff will not assist with the supervision of children prior to obtaining criminal record clearance and associated to the facility. Based on record review and interviews conducted it was revealed that staff #2 who was not fingerprint clear and associated to the facility assisted staff #3 with the supervision of children during outdoor activities. This is a Type A deficiency which poses an immediately health and safety risk to children in care. 8

This requirement was not met as evidence by: 1 Director to conduct staff training concerning bathroom procedures to ensure that there is staff visual observation when children use the bathroom. A copy of the training attendance sign in sheet with that staff that participated in training shall be submitted to the Department no later than 07/21/22. During interviews conducted with staff it was revealed that staff stand outside the bathroom when children need to use the restroom not directly supervising the children which resulted in Child #1 having inappropriate contact with child #3. 8

Both staff members were removed immediately from the facility. Based on observation and record review, 2 staff members were not fingerprint cleared and were caring for children. This poses an immediate health and safety risk to children in care. 8
