Based on interview and record review the day care center was operating out of ratio in the morning hours from March 16, 2026 until March 18, 2026 which is a potenital health, safety, or personal rights risk to the persons in care.
Based on observation and interview the licensee did not comply with the section cited above in the milk being served to the day care children two years and older was observed to be 2% milk and the representative was unaware which poses a potential health, safety or personal rights risk to persons in care.
Based on interview and record review Staff left Child 1 (C1) alone unsupervised in the hallway while transitioning to another classroom a parent observed C1 alone in the hallway and took C1 to their classroom which is an immediate health, safety, or personal rights risk to the persons in care.
Based on observation, interview, and record review, the licensee did not comply with the section cited above in child/parent roster was not available during the inspection which poses a potential health, safety or personal rights risk to persons in care.
Based on observation Staff #1 and staff #2 does not have proof of fingerprint clearance, which poses an immediate Health, Safety or Personal Rights risk to children in care.
