Based on record review, the licensee did not comply with the section cited above in Staff #1 and Staff #5 did not have a TB test in the file, which poses/posed a potential health, safety or personal rights risk to persons in care.
Per Director, she will provide an LIC624 concerning this incident no later than 05/08/23 via email to the Department. On our about December 2022, an infant and staff member was diagnosed with hand, foot, and mouth disease. The facility failed to report the incident within 24 hours, and did not submit an Unsual Incident Repport within 7 days of the occurrence. This is a type B deficiency, that if not corrected, it poses a potential risk to the health and safety to children in care.
