Licensee has already attended an informal meeting with Regional Manger A Juarez and LPM Iglesias on 3/29/22, via Zoom meeting was informed that one food group with water does not meet Title 22 requirements, however, licensee continued to serve snacks with one food group with water. Licensee informed staff to provide another food group when Licensing was present in facility. This is an immediate personal, health and safety risk to children in care.

This requirement was not meet as evidence by LPA's observation of 1 Per Jessica Ramsey, she is the new director, she will provide a copy of detailed training agenda and attendance to the Fresno Community Care Licensing office no later than 6/17/22. This is the second same citation issued to licensee. A civil penalty of $250.00 has been assessed. Licensee is to pay the bill upon receipt. licensee using an aggressive communication style when raising her voice to be heard across the classroom when speaking to day care children. This is an immediate personal rights, health and safety risk to children in care. This is the second incident that LPA observed in facility. 8

The facility can no longer provide day care services to the infant. This infant is to be picked up by parents immediately. The infant cannot return to center. Licensee shall provide proof of infant no longer attending facility. Licensee shall provide documentation on how she will no longer bring children, facility, receiving care and supervision by facility staff. The front door and 1 Per director, all staff will attend training that reviews Care and Supervison to children in care; and what that entails. A copy of detailed training and attendance will be sent to Fresno Community Care Licensing office no later than 5/23/22. classroom door have alarms to alert staff of the doors being opened. During today's visit, Licensee provided LPA with a written statement, indicating she will not use foul language when addressing issues with staff and will be more conscious of words used to describe staff/ an individual. Additionally, a "Zoom" meeting will be scheduled with Licensee, LPA's Supervisor, and LPA to address complaint(s). On 12/7/21, LPA interviewed Ms. Pershall and during the interview, she admitted to using inappropriate language while addressing an issue with a staff. Also, during the interview Ms. Pershall used language that can be offensive to describe an employee. This is a potential personal rights, health and safety risk to children in care.

Administrator will be providing LPA with a detail agenda of the topic discussed, along with a sign in/sing out sheet of those in attendance. This requirement is not met as evidence by: Administrator admiting that staff were not monotring children at all times, resulting in a child sratching another child, and staff not having knowledge of the incident until the following day. 8 Administrator is to mail out the agenda and sign in/ sing out sheet to CCL by 10/13/2021.
