The facility owner stated that she would create a letter for parents, notifying them of the regulations/policies/procedures for the center in relation to capacity and teacher/child ratios. She agreed to follow all regulations in relation to Teacher-Child ratios. Parent letter The facility owner self-disclosed that the facility cared for thirty (30) children with only two staff members on one occasion. This poses a potential risk to the health, safety, or personal rights to children in care. 8 to be submitted to the Department by the Plan of Correction due date, September 9, 2026.

The Director stated that all staff will be retrained on how to provide appropriate supervision to children in care, will provide proof of the the training curriculum, along with proof of participation for staff by the Plan of Correction date, July 24, 2026. Per interviews with staff, it was revealed that due to inadequate supervision, there were two (2) reported incidents where two (2) different children in care reported being touched inappropriately by other children on their private areas. This poses a potential health, safety or personal rights risk to persons in care.
