(2) Bedding shall be individually stored so that each child's bedding is identifiable and no child's used bedding comes into contact with other bedding. This requirement was not met by: 1 Director states that they separately stored the napping equipment on 8/30/24 when new staff took over Room 10. LPA observed the correction on 10/28/24. Based on observation on 8/26/24, LPA observed napping mats touching/stored together in a preschool classroom which is a potential health and safety risk to children in care. 8
… Based upon interviews with staff and parents and documents reviewed from the facility and complainant, staff member S1 was alone with 5 children in a classroom and was not visually 1 In classrooms where a child that is a known biter is present, staffing should include an extra staff member to help with supervision of the child that bites to prevent it from occurring. Director stated she will provide a training on how to prevent biting in the classroom and to inform staff members to ask for help when needed. supervising the other children while changing a diaper (her back was turned), when C1 bit another child (C2) for the second time twenty minutes after the first bite which could have been prevented with adequate staff supervision. This is a potential health, safety and personal rights risk to children in care. 8 When help is requested in a classroom due to supervision needs and/or biting, Director will send a staff member or herself to help in the classroom immediately. Director stated she will provide the training by 2/23/24 and e-mail the staff signed/dated meeting agenda to…
… 1 Director shall include a training by 2/23/24 that includes directions to staff members on how to fill out and deliver incident reports for parents. Incident reports shall be delivered and communicated to parents same day of the incident at pickup. Based upon interviews with Director, staff, complainant and parents, parent of C2 was not notified of the second bite upon pickup from the center. Other parents also stated that notification of injuries and behavior issues (biting) are not consistently communicated or received from the facility which is a potential health, safety and personal rights risk to children in care. 8 Incident reports should be consistently written for both the biter and child that was bitten and records kept to know which child was bit and when. Director will provide a signed staff agenda dated by 2/23/24 and email to LPA Lane by EOB on 2/23/24.

Based on file record review, the licensee did not comply with the section cited above in that Staff #4 did not have Pertussis or Measles immunity/vaccination information on file and Staff #6 did not have Measles immunity/vaccination information on file, which poses a potential health, safety or personal rights risk to the health and safety of children in care.
Based on file review and observation by LPA, staff Kadija Osman who was supervising in Room #3 did not have a fingerprint clearance associated to the facility.
LPA's interview with staff. On 12/16/21 a child sustained an injury that required a dental visit. Child sustained injury to the gum area and superficial cut on the cheek. Director failed to report this incident to the department.
Based on file review and observation by staff Tyra Block on 10/20/21, staff Jessica Wheeler was present in Room #2. Jessica Wheeler did not have a fingerprint clearance associated to the facility. Site Director Jennifer Low was present today. Her fingerprint clearance is not associated to the facility. Ms. Low was hired in August 2021.
Based on observation the rubber matting on the playground to the right has a dip/ sinking spot that poses a potential health and safety risk to children in care.
