Staff stated they can get it at home and will provide to LPA the next day. Staff also does not have Mandated Reporter Certificate which poses a potential health, safety or personal rights risk to persons in care.

101700.3 (b) Testing results with fractional ppb readings of 0.5 ppb or greater shall be rounded up to the nearest whole number, before comparing to the Action Level. (1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. During visit, LPA confirmed lead results were posted, and lead documents were submitted. Director will remove, replace or repair outlet P& Q and will submit plan of correction by 02/20/2023. This requirement is not met evidenced by record review of the lab report analysis from Outlets K(ALE 7 rm 10 ) & N(ALE29 kitchen) taken on 12/03/2022. Results indicated levels of lead in exceedance at 5.6 This poses a potential risk to the health and safety of children in care. 8

(a) The licensee shall ensure that each child is accorded the following personal rights: (3) To be free from corporal or unusual punishment, infliction of pain, humiliation ………. Per Acting Director, will write a memo staing proper child guidance and Personal Rights regulations. Per Acting Director, employees signature will acknowledged each staff understood the information included in the memo. Per Acting Director, she will email each staff signed memo to LPA by due date of 02/04/2022. This requirement is not met as evidence by: Based on interviews, C2, C3, and S1 stated S2 hit C1’s arm. C2, C3, and C4 stated they S2 hit C1 on the arm four times. This poses an immediate risk to the safety of children in care.
