Based on LPA observations, record review, interviews, and by the licensees own admission, evidence indicates a subject staff yelled at the preschool while supervising children, which is a potential risk to the health and safety of persons in care.
Based on interviews and record review, it was determined that staff administered the wrong medication to a child in care, resulting in the child being taken to the doctor as a precaution. This posed an imediate health and safety, and personal rights risk to the child in care. 8 The facility agrrees to submit documentation as proof of completion to LPA by 10/20/25.

It was determined that a preschool child's class transitioned into their classroom and one child was left in the breezeway playground. However, the subject child was observed by a nearby school-age teacher for the whole time until the child was walked back to their class. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). (a) The licensee shall ensure that each child is accorded the following personal rights: (2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. This requirement was not being met as: 1 The facility agrees to submit a statement ensuring the Department how they will provide children in care comfortable accomodations moving forward. The facility agrees to submit the statement to LPA via email by 01/18/2024 by 5:00 PM. During the investigation process, it was revealed that the subject classroom had a temperatue of 58 degrees fahrenheit. This was due to the facility's heating was not working. This took place on 01/09/2024 and 01/10/2024. The facility's heating system was fixed on 01/11/2024. 8
(g) Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children shall be stored where inaccessible to children. (1) Storage areas for poisons shall be locked. This requirement was not being met as: 1 Staff N.1 locked all the cabinets during this inspections. Assistant Directors agress to purchase additional latches. Work order and/or pictures of purchase and replacement on the cabinets must be submitted to CCL by deadline. cleaning supplies were observed by LPAs in the lower shelf of the upper cabinet unlocked as teachers arrived and left their purses on shelf upon arrival to the classroom. Classroom: Preschool and Discovery Preschool. This posed a potential health and safety risk to children in care.
Based on interviews, staff did not ensure they were applying the correct diaper rash cream on children in care causing one child to be in severe pain and unable to walk. Additionally, due to infrequent diaper changes and/or a child not being cleaned properly, contributed to at least two children’s diaper rashes, which poses an immediate Health, Safety, or Personal Rights risk to children in care. 8 staff. A written statement will be submitted to CCL by the next business day on 6/27/2022 detailing how the deficiency has been corrected, including the revised diapering procedures.
